Permission to Skate and NRP (Passport) Forms (Erin-Hillsburgh Minor Hockey Association)

PrintPermission to Skate and NRP (Passport) Forms
Permission to Skate and NRP (Passport) forms are provided to the EHMHA by the OMHA in late March / early April.

Erin-Hillsburgh players can request one or both of these forms using the PTS / NRP request form linked here.

If you have more than one child who needs a form(s), please fill out a separate request form for each child.

PTS forms will be e-mailed;  NRP forms will be distributed once they have been received from the OMHA..

There is no cost to get one or both of these forms, and all forms requested will be given.

PTS (Permission to Skate form)

Players interested in trying out for their AAA zone team require a Permission to Skate (PTS) form from their home centre (Erin-Hillsburgh).

The AAA zone team for Erin-Hillsburgh players is the Halton Hurricanes.  The PTS form issued by EHMHA is to tryout for the Halton Hurricanes.  In order to tryout out for a different AAA team, Erin-Hillsburgh players would first need to be released, and get an F1 waiver form from Halton Hurricanes.

In 2020, AAA tryouts start on April 13th.

NRP (Non-Resident Passport)

Erin-Hillsburgh players going into U12 (Minor Peewee) or older age groups are eligible to tryout for one AA or A centre.

Players interested in trying out for a AA or A team need to request an NRP, issued by EHMHA.  Only one form per player can be issued; players can only attend one tryout (AA or A)
