PTS/NRP Request Form for 2024/25 Season
1) Please fill out form below to request PTS/NRP forms to attend tryouts at other centres for 2024/25 Season. 2) After completing the form below, log into your Hockey Canada account and register there by selecting the PTS/NRP item in the registration menu: https://page.spordle.com/erin-hillsburgh Forms will be sent as soon as OMHA portals are open - please DO NOT email asking for your forms, they will be sent as soon as possible.
If you have more than one child who needs a form(s), please fill out a separate request form for each child.

A PTS (Permission to skate) form is required to tryout for AAA. 
 
A NRP (Non-Resident Passport) form is required for U12 and above players interested in trying out for one AA or A team. These are now available digitally. You will be emailed the form at the end of April.

If the player is released from the AA or A centre they tryout for they will return to their home centre, they will not be given another NRP form.

The earliest you will receive NRP forms is end of April, Do NOT email us asking for your forms, they will be sent once available.
