    This form is for payment only - it is not an order form.

    Jerseys and socks will not be distributed to players without payment via this form. Once completed, a copy of this form will be sent to the equipment director as proof of purchase.

    Please note we are having some technical issues with the Paypal portion of this form.  The form has been temporarily disabled, and if you have not been able to submit payment we will contact you once it is working again! Thanks for your patience!
