23/24 Travel Permit Request

Use this form to request a travel permit - an official travel permit will be requested from the OMHA by the VP Admin.

Upon receipt of this request, the exhibition game or tournament will be entered into the schedule by the ice scheduler as soon as no ice conflict is confirmed; note that the event cannot be considered official until the OMHA travel permit has been received, even if it has been entered into the team schedule.

Off-ice events should be entered into the schedule by team manager and do not require a travel permit.
