PTS / NRP Request Form
If you have more than one child who needs a form(s), please fill out a separate request form for each child.

A PTS (Permission to skate) form is required to tryout for AAA.
The Erin-Hillsburgh AAA home centre is Halton Hurricanes regardless of the location of the players residence.
 
A NRP (Non-Resident Passport) form is required to tryout for the nearest AA or A centre to your residence.   The NRP is only available to players in PeeWee or older age groups.

If you requested a PTS form, it will be e-mailed to you within the next week (before Apr 13th)

If you requested a NRP form it can be picked up on Tues Apr 21, 6:45 pm - just before the AGM.
