NEW TRAVEL PERMIT PROCESS

Use this form to request a travel permit.

Ice events (exhibition games / tournaments) will be entered by into the schedule by the ice scheduler after they first confirm there is no ice conflict.
An OMHA travel permit will then be requested by the VP Admin.

Exhibition games and tournaments should not be considered approved until you have received a copy of the OMHA travel permit from the VP Admin or their backup, even if the event appears in your schedule.

Off-ice events should be entered into the schedule by team manager.
